How Start the Process!

Step One: We Need to Establish an Account for You
In order for your facility to be certified, you will need to complete a simple application. To do so, we must first create an account for you.
Once we have established your account, you will receive an email from us entitled: "Your CLHF Account". Please save this email for future reference. Check your spam filters or junk mail in case your facility's email system does not recognize us. If you do not receive an email from us within 48 hours, please contact Debbie Benson at dbenson@CIHQ.org for assistance.
Step Two: Log Into Your Account and Complete Your Application
Login to your account, complete and submit your application for certification.
